On March 4, 2025, CardioVascular Health Clinic (“CHC”) experienced a network disruption. We immediately began an investigation, which included working with third-party specialists. Our investigation determined that certain information stored on our network was accessed by an unauthorized individual between February 18, 2025 and March 4, 2025, as a result of a security incident affecting our cloud services provider. Although CHC is unable to confirm the specific information that may be affected at this time, they are providing notification in an abundance of caution as they value the security of information. The type of information potentially impacted will likely vary by individual but may include name and one or more of the following: name, address, phone number, email address, date of birth, Social Security Number, driver’s license/state ID number, financial account information, treatment/diagnosis information, prescription information, provider name, medical record/case number, Medicare/Medicaid ID number, health insurance information, and treatment cost. We are currently reviewing the data related to potentially impacted individuals and will be providing written notice to individuals for whom we have addresses.
In response to this incident, we notified law enforcement and reviewed our policies and procedures. We are also providing individuals access to credit monitoring and identity protection services as an added precaution. In general, we encourage individuals to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors. Under U.S. law, individuals are entitled to one free credit report annually from each of the three major credit reporting bureaus, TransUnion, Experian, and Equifax. Individuals have the right to place an initial or extended fraud alert on a credit file at no cost. If individuals are a victim of identity theft, they are entitled to an extended fraud alert lasting seven years. As an alternative to a fraud alert, they have the right to place a credit freeze on a credit report. The credit freeze is designed to prevent credit, loans, and services from being approved without consent. Pursuant to federal law, individuals cannot be charged to place or lift a credit freeze on your credit report.
