Understanding Nutcracker Syndrome

The left renal vein — which drains blood from the kidney — lies near two major blood vessels: the abdominal aorta and the superior mesenteric artery. Nutcracker syndrome occurs when the renal vein becomes compressed between those two blood vessels, restricting the blood flow and causing pressure to build up in the vein. The name “Nutcracker” comes from the way those two blood vessels squeeze the renal vein, much like how a nutcracker might squeeze a walnut to crack it.

A variety of symptoms and complications can arise from the increased pressure. These vary from person to person, and because of its rarity and range of symptoms, Nutcracker syndrome can sometimes be hard to identify and diagnose. 

If left untreated, Nutcracker syndrome can eventually cause damage to the left kidney, resulting in decreased kidney function or even kidney failure. Additional complications from this can include blood clots, pulmonary embolism, or death.

Here are some of the most common signs and symptoms of Nutcracker syndrome.

  • Abdominal, Back, or Flank Pain: A common symptom is pain on the left side of your abdomen or back, often near the kidney (the lower part of your back, below your ribs). This pain can range from mild to severe and activities, like more strenuous physical exercise, may make it worse.
  • Blood in the Urine (Hematuria) or Protein in the Urine: Sometimes there may be blood in your urine. There might be microscopic particles which are not visible to the naked eye, or more noticeable amounts. For some people, hematuria may come and go, while other people may consistently notice blood in their urine.
  • Pelvic Pain and Pressure: In women, Nutcracker syndrome may cause pelvic discomfort — especially around menstruation. This is because of the increased pressure in the veins. Women with renal vein compression may also have irregular or heavy menstrual cycles. 

These are also indicators of pelvic congestion syndrome, which, although a condition on its own, can also be a symptom of Nutcracker syndrome, contributing to irritable bladder or bowels or a dull ache in the pelvis, especially when standing for long periods or during the menstrual cycle.

  • Varicocele in Men: In men, Nutcracker syndrome can lead to a left-sided varicocele, which is a swelling of the veins in the scrotum. This is caused by the backup of blood because of the vein compression. 
  • Fatigue and Other Symptoms: Some other symptoms of Nutcracker syndrome include chronic fatigue, dizziness when standing, varicose veins, and digestive issues.

  • Rapid growth during puberty
  • Severe lower spine curvature
  • Pregnancy
  • Rapid changes in height or weight
  • Low body mass index (BMI)
  • Enlarged lymph nodes in your abdomen

Because of the structural causes, Nutcracker syndrome isn’t typically “preventable” in the same way that lifestyle-related conditions are. However, learning about the symptoms to watch for and the causes can help you and your doctor evaluate if renal vein compression is the source of your discomfort.

Diagnosing Nutcracker Syndrome

Because many of the symptoms associated with Nutcracker syndrome are similar to those of other health conditions — such as kidney stones or bladder infections — diagnosing it may require multiple steps. 

Your doctor will start with a thorough review of your symptoms and medical history. However, to confirm renal vein compression, they will need to rely on imaging tests, which may include:

  • Ultrasound. A Doppler ultrasound is a non-invasive test that uses sound waves to visualize what is happening inside your body. Your doctor will be able to see the blood flow in the renal vein and can identify if any areas are compressed.
  • CT or MRI Scans. These scans create detailed images of the blood vessels and the surrounding tissues. They let doctors see if there is any compression and precisely where it may be.
  • Venography. A venography is a special type of X-ray that allows doctors to see the blood flow in the veins by injecting a dye. Venography can help identify the exact location of a compression and how severe it is.

Treating Nutcracker Syndrome

The treatment approach for Nutcracker syndrome recommended by your doctor may vary depending on how severe your symptoms are. Mild cases of Nutcracker syndrome may not require much intervention, but more severe cases might be treated with a surgical approach.

Possible treatment options for Nutcracker syndrome may include:

  • Conservative management. When the compression or symptoms are mild, doctors may recommend monitoring your symptoms without immediate interventions. They may recommend lifestyle changes, such as maintaining a healthy weight, to relieve some of the pressure on the renal vein.
  • Pain management. Sometimes, doctors may prescribe medication to help manage pain and inflammation. Although this doesn’t address the underlying compression, it can make symptoms more manageable.
  • Endovascular stenting. When conservative management doesn’t help improve symptoms, your doctor may recommend a minimally invasive procedure called stenting. This procedure involves placing a small metal tube (stent) in the compression section of the renal vein to hold it open. This allows the blood to flow more freely through the vein.
  • Surgical options. Surgery might be recommended in more severe cases. Some of the surgical options include bypassing the compressed portion of the vein or repositioning it to relieve the pressure on it. Although surgery is generally only recommended for persistent cases, it can be very effective at restoring proper blood flow.
