AV Fistulas

An arteriovenous fistula (AV fistula), or dialysis fistula, is a surgically created connection between an artery and a vein in the arm or wrist, providing a durable, long-lasting access point for hemodialysis. AV fistulas are preferred for long-term dialysis because they are less prone to infection and generally outperform other types of access, like catheters, in durability and longevity.

CardioVascular Health Clinic: Leading in Cardiovascular and Renal Care

CardioVascular Health Clinic is a leader in cardiovascular health and offers innovative, minimally invasive procedures designed to improve patient outcomes and quality of life. Our multidisciplinary team—including cardiologists, vascular surgeons, and interventional radiologists—works collaboratively to prioritize your health. We understand the vital role AV fistulas and grafts play for patients with kidney failure, which is why we emphasize long-term care that goes beyond fistula creation. Our commitment includes continuous maintenance, coordination with external specialists, and comprehensive cardiovascular care to support your overall health.

The Importance of AV Fistulas in Renal Care

For patients with advanced kidney failure or end-stage renal disease, hemodialysis is essential to replace kidney function. This process involves removing blood from the body, filtering out waste, salts, and excess water, and returning cleansed blood to the bloodstream. Because typical veins cannot handle the high blood flow and frequent needling required for dialysis, a stronger vessel is necessary. Surgically created AV fistulas provide these "super veins," capable of handling dialysis demands while preserving blood flow and functionality.

How AV Fistulas Improve Dialysis Effectiveness

Creating an AV fistula involves connecting a vein to an artery. This connection allows faster blood flow through the vein, bypassing capillaries and causing the vein to thicken and enlarge over time. This "super vein" becomes strong enough to withstand regular needling for dialysis and provides the blood flow needed for effective treatment. With an AV fistula, both dialysis needles can be inserted directly into this strong vein, reducing the need for plastic lines and potentially shortening dialysis times.

The CardioVascular Health Clinic Difference in Fistula Care

At CardioVascular Health Clinic, our comprehensive, whole-person approach sets us apart. Unlike many vascular centers that focus only on fistula creation, our team provides ongoing maintenance and support throughout your dialysis journey. Our physicians collaborate closely with your nephrologist, offering routine maintenance and ultrasound surveillance to ensure that your fistula remains functional and safe for dialysis.

Our multidisciplinary team recognizes the connection between kidney disease and cardiovascular health. Patients with kidney failure often face heightened risks of peripheral artery disease, coronary artery disease, and limb loss. With advanced diagnostics and treatment options, we monitor and address these cardiovascular risks, helping you maintain overall health.

The Fistula Procedure at CardioVascular Health Clinic

Our fistula procedure is performed in an outpatient setting, where you can expect a lower cost and reduced risk of complications compared to hospital surgery. The procedure involves the following steps:

  1. Anesthesia. You’ll receive local or general anesthesia.
  2. Incision. The vascular surgeon makes a small incision to access the selected artery and vein, which are stitched together.
  3. Closure. The incision is closed with stitches, glue, or tape.

After surgery, your fistula needs several weeks to mature, allowing the vein to thicken and strengthen for dialysis use. This process may take up to a month or longer.

FAQs

What are the benefits of a fistula for dialysis?

AV fistulas are the preferred access for hemodialysis because they:

1. Are more durable and longer-lasting than other access types.
2. Have a lower risk of infection and complications, making them easier to clean and maintain.
3. Are internal, eliminating the need for external tubes, allowing patients to bathe, shower, and swim comfortably.
They also improve dialysis efficiency by increasing blood flow and facilitating more effective impurity removal.

Am I a candidate for a fistula?

To qualify for an AV fistula, you need an adequately sized vein and artery in your arm or leg. Our specialists will perform a physical exam and vein mapping via ultrasound or angiogram to evaluate the size and health of your blood vessels.
If they’re suitable, we’ll schedule your fistula procedure.
If your veins are not suitable, you may still qualify for an AV bypass graft, which uses a synthetic or bovine graft to connect the artery and vein indirectly.

What is the difference between an AV fistula and an AV graft?

Both AV fistulas and grafts connect an artery to a vein for dialysis access. Key differences include:
1. An AV fistula is a direct connection; an AV graft uses a synthetic or bovine graft to create an indirect connection.
2. AV fistulas take 6-12 weeks to mature; AV grafts are ready for use in 2-3 weeks.
3. AV fistulas are the preferred option if suitable veins are available; AV grafts are used when veins are too small or weak for a fistula.

Testimonials

I’m about one month out and things are so much better. At my two week evaluation my symptom score had gone from a 27 to a 7, and I’m sure it’s even better now. That speaks volumes. Now that I’m post recovery, there is no incontinence, I go to the bathroom much less and there are no ED side effects like there are with other procedures. In regard to my quality of life, well, I don’t maintain my life based on my proximity to a bathroom anymore.

Professional staff working in a beautiful facility with great doctors who sincerely care about their patients. Without Dr. Schmidt's skill, I would have needed heart bypass surgery instead of a stent. Follow-up has been thorough. I can't recommend them highly enough.

I came to Dr. Warren by a self referral for mals when it took me 8yrs 50+ doctors and thousands of dollars spent on no answers. My first visit I was scared I was just going to be gaslighted just like the past however I was pleasantly shocked when Dr. Warren immediately believed me. She took the time to review many disk I had brought from past tests. She never once dismissed me and listened to ever word I had to say. She scheduled one last test to confirm the diagnosis then I was scheduled for surgery right away. Surgery was a success and I can’t thank her and her staff enough. Dr. Warren truly saved my life.
