Office Hours

  • Office hours are Monday through Friday, 8 am to 4:30 pm.
  • Our doctors share coverage with each other on an alternating basis after hours during the week and weekends, assuring you continuity of care with providers available to you 24 hours a day.

All Patients

If you are a new patient, please arrive at least 30 minutes prior to your appointment time in order to get your information registered into our system. If you are a follow-up patient, please arrive 10-15 minutes prior to your appointment time. Please bring the following items for your visit:

  • Completed new patient forms
  • Current driver’s license
  • Current health insurance card(s)
  • All your current medications (in the original containers), including herbs and dietary supplements
  • Your primary care physician and/or referring physicians name and phone number
  • Current medical records, including any recent diagnostic testing
  • You may download forms prior to your visit by clicking the patient tab

Medication Refills

If you need a refill of your medications, please call your pharmacy. They will send a refill request to the office. If you call the office with a refill request. Please be ready to leave a message on the nursing line with your name, date of birth, medication dosage and how you are currently taking it. Also provide the name, location and phone number of your pharmacy. Routine medications will be refilled in 24-48 hours. Medications will not be refilled after hours, on weekends or holidays. Please plan ahead so that you do not run out of your medications.


In case of non-life-threatening emergency, please contact the office 24 hours a day. Our answering service will connect you with the physician on call. For any life-threatening emergency, call 911.


If you have a question feel free to call the office between 8 a.m. and 5 p.m. Our nurses and staff are trained to answer questions and pass information between you and your physician. We will make every attempt to return calls by the end of the business day.
